To book an appointment follow these steps:
(1) Fill out the Intake Form below.
(2) Use the scheduling link to pick a date and time for your appointment.
Please complete the form and provide as much detail as possible.

We ask that you please reschedule or cancel at least 3 days before the beginning of your appointment or you may be charged a cancellation fee.
All scheduled appointments booked will be charged $75.00* if cancelled or no show within 72 hours. *$75.00 fee is for regular scheduled appointments under 4 hours. All scheduled appointments booked for any All Day Deals (1 person or 2 people) at 8 hours will require $250.00** charge if cancelled or no show within 72 hours. **$250.00 fee is for All Day Deals and or 6-8 hours scheduled appointments. All scheduled appointments booked for more than 4 hours but no more than 6 hours will be charged a fee of $150.00 if cancelled or no show within 72 hours.
